Definitions

• Existing Patient: An individual who has previously established care with ReCenter Medical or ReCenter OASIS and has completed at least one paid service and has a record under ReCenter’s EMR.

• New Patient: An individual who has never previously received services at ReCenter Medical or ReCenter OASIS and completes an initial qualifying treatment; or is a reestablishing patient.

• Referral: A New Patient who identifies an Existing Patient at the time of scheduling or prior to checkout and establishes care under an eligible promotional service.

Eligibility & Redemption

• Referral rewards are issued only after the referred New Patient completes their initial qualifying service and payment is received in full.

• Existing Patients may claim referral incentives only once the referred New Patient has officially established care and received treatment under one of the listed eligible services.

• All offers are non-transferable, have no cash value, and may not be combined with other promotions, packages, memberships, or discounted pricing unless otherwise stated.

• Discounts, rebates, and complimentary units apply to treatment fees only and exclude consultation fees, lab fees, taxes, and ancillary services unless specified.

• All services are subject to provider evaluation, medical eligibility, and clinical appropriateness. Individual results may vary.

• Promotions are valid for a limited time and may be modified or discontinued at the discretion of ReCenter Medical and ReCenter OASIS.

• ReCenter reserves the right to verify eligibility and deny promotional application if terms are not met.
